Hospice of the Valley helps improve quality of life for people with all stages of Alzheimer’s disease and other dementias—from those just starting to show signs to those who are quite advanced. Our experienced dementia team is here to support you every step of the way on your caregiving journey and can work with your loved one’s physician to create a customized plan of care.

We offer in-home education and support for dementia caregivers at no charge, as well as compassionate care for loved ones who need dementia hospice care. Our world-class Dementia Care and Education Campus provides comprehensive support for all types and stages of the disease, from an Adult Day Club and Assisted Living Center to community classes and memory café groups.

Enhanced Dementia Support

Medicare has selected Hospice of the Valley’s Supportive Care for Dementia program to provide a new model of care called GUIDE (Guiding an Improved Dementia Experience). The goal is to improve access to quality dementia care and support more caregivers, including respite services to give family members an occasional break.

“The new GUIDE Model helps families care for their loved ones at home and reduce the stress related to being a caregiver,” said Hospice of the Valley CEO Debbie Shumway. “Arizona has one of the fastest-growing rates of dementia in the nation, so this support is critically needed, and it is our honor to provide it.”

Dr. Gillian Hamilton is medical director of Supportive Care for Dementia and GUIDE programs. “This is a beautiful opportunity to expand our care for patients at all stages of the condition long before they need hospice care,” she said. “This model will demonstrate nationally the vital impact this kind of support has on quality of life. We look forward to supporting more people with dementia who live alone and helping family members navigate caregiving challenges.”
